What is Circumcision?

As the summer season approaches, “What is circumcision?” “When should we do it?” more and more questions are being asked. The excitement of circumcision begins for parents of boys. Especially for children who go to school, the summer season is a preferred period for circumcision. While some families prefer circumcision at an early age so that the child does not have any worries about circumcision in the future and does not have any memories about circumcision, some families, on the contrary, think that circumcision should be an important memory for the child and want to experience circumcision in a more ceremonial atmosphere.

What is circumcision?

Circumcision is one of the oldest and most frequently performed surgical procedures all over the world. The beginning of male circumcision is thought to date back to the earliest periods of human history.

Circumcision means cutting and removing the foreskin at the tip of the penis. Today, about 1/6 of the world's male population is circumcised. It is known that there are stronger data on the history of circumcision in Ancient Egypt around 2400 BC.

What is Newborn Circumcision?

Recently “What is newborn circumcision?” The question "What is circumcision?" It was asked more than a question. To explain the newborn period first; The neonatal period covers the range of 0-28 days from birth. Circumcision is also possible during this period. Circumcision in the newborn period is highly preferred, especially in the last period. Because in this period, the care and wound healing of the circumcised baby is much easier than in other periods.  The most important benefit of newborn circumcision is that it can be performed under local anesthesia and the tissues can heal faster. The circumcision wound, which heals in a week after the age of 6, heals in 2-3 days in newborn circumcision. The knowledge that babies do not feel pain in the neonatal period has now been replaced by the knowledge that "He feels pain, but cannot perceive it adequately". Therefore, it is absolutely not correct to perform circumcision without anesthetizing.  Breastfeeding is very useful for pain control in newborns. Breast-feeding; It is a whole that includes sensual contact, mother-infant communication, sensory receptors on the skin and activation of the sense of taste. Thus, the child's drug use can be reduced with the benefit of breastfeeding.

We passed the newborn period, when can we get circumcised?

Unless there is a concomitant disease, there is no inappropriate time for circumcision in terms of physical health. However, if we have passed the newborn period, it is psychologically more appropriate to have it done until the age of two, and if this period has passed, to wait for the age of six. The reason for this is that children cannot fully understand the events in this period and it is difficult to communicate. In this age range, children may think that they will be harmed during circumcision, and this fear may cause them to experience 'castration' (castration) fear in the future. However, this is not true for every child.  If you think your child is psychologically ready for circumcision, circumcision can be performed during this period as well. Some children may not be psychologically ready for circumcision even at the age of 7. On the contrary, some children may be ready at age 4.

No matter what age the child is before circumcision, it is very important to prepare the child well and to do this with the right sentences. “What is circumcision?” The answer to the question should be chosen in a way that the child can understand and does not frighten the child. Children should not be deceived or suddenly compared to the doctor. It is very important not to go into details when explaining the circumcision, and to explain this process with words such as "cleaning, curling, removing excess skin". Having the child watch the circumcision videos published on the internet before the circumcision, and explaining the process as if it is done with unrealistic tools for entertainment creates deep fears in children and these fears create negative situations that are difficult to compensate for in the child's later sexual life. It should not be forgotten that children are “children” no matter how mature they are.

Circumcision is an event that your child will experience once in his life and will significantly affect his future life. Therefore, when deciding on circumcision, a good decision should be made in terms of the time of the procedure, the place to be done and the person who will do it. Circumcision is a surgical procedure. All surgical procedures should be in sterile conditions. Circumcision should be performed by pediatric surgeons and pediatric urologists who are specialized in this field.

Local Anesthesia or General Anesthesia?

It can be done easily with local anesthesia (regional numbing) in the first months after the baby is born. In older children, the choice of local anesthesia-general anesthesia should be made with the decision of the family and the physician, depending on the condition of the child.  Local anesthesia can cause serious psychological trauma in older children. Therefore, the type of anesthesia should be decided according to the condition of the child.  The general anesthesia method used for circumcision is sedation anesthesia and it is a mild anesthesia method. You may feel pain during the procedure. Therefore, in addition to sedation anesthesia, local anesthesia is also used. If circumcision will be performed outside the hospital, in the clinic or at home, the type of anesthesia applied is always local anesthesia.

In which season should circumcision be performed?

All seasons are suitable for circumcision. There is no specific season that we recommend. However, the summer period can be preferred for school children in order that the process to be done does not prevent the disruption of the child's education and that other friends are more easily convinced by seeing that it is done in this period, and that they think that this process is a normal procedure.

Which Methods Are Used in Circumcision?

There are many methods for circumcision. The main ones are:

  • Thermocautery/ Laser Circumcision: It is done with a device known as a circumcision soldering iron. Due to the low risk of bleeding and the short duration of circumcision, it is highly preferred by circumcised people. However, its reliability is controversial because it can cause nerve damage and sexual dysfunction after circumcision.

  • Clamp/ Clamp Method: In this method

The inner and outer skin is literally pinched by means of a plastic ring, the excess skin is cut off, and the remaining skin with impaired circulation also falls off on its own by gangrene. The clamp needs to be removed a few days after the procedure. During this time, the child must remain in the penis, and this can be uncomfortable for children. Extraction takes a very short time. However, for children, doing a new procedure is still unsettling.

  • Surgical method / Classical method: It is the most common and most reliable method. However, if it is not done by the right people, it can have sad consequences. After the foreskin is cut, it is sutured with self-absorbable sutures. For babies with diapers, there is no harm in tying the diaper, holding the baby on one's lap, or lying on his stomach.

Is Circumcision Performed at Home?

Circumcision can be performed at home if suitable conditions are provided. What is important here is the provision of an adequate sterile environment and the circumcision experience of the circumcised person. Again, circumcision can be performed at home in the newborn period.

What Are the Possible Problems After Circumcision?

The most feared complication is bleeding. However, if it happens with the bleeding drop style, it should be taken care of. In this case, the surgeon who performed the circumcision should be contacted. This may be due to inadequate bleeding control. However, it can also be caused by certain blood diseases or trauma due to various drug use. Leakage, spotting-like bleeding may occur. For these, there is no need to do anything unless there is an increase in the amount.

The probability of infection is very low. If adequate hygiene is provided, infection is not expected. One of the most frequently asked questions is the possibility that diapers can pave the way for infection. Infection does not occur with regular diaper changes. At the same time, the gland can be useful in terms of protection from trauma.

After circumcision, there may be swelling in the penis, bruises due to local anesthesia.

, should not be worried. In case the foreskin is adhered to the head of the penis, a thin layer of skin may be peeled off due to the opening of this adhesion during the procedure, and this leads to redness in the early stages of circumcision and then crusting. In children with diapers, yellow-white plaques may occur due to the moistness of these crusts. This condition can be mistaken for inflammation. However, all these mentioned situations are normal.

Where Is Mesotherapy Used And Which Agents Are Used?

Mesotherapy cellulite, face and neck rejuvenation, skin care (skin sagging, wrinkles, tension lines, providing skin tone), regional slimming, lipolysis, hyperpigmentation, alopecia, acne, pruritis, hyperkeratosis, psoriasis, telangiectasias, scars, migraine headache, tendinitis It is applied in cases such as arthritis, sports injuries. There are many techniques in mesotherapy application. However, the choice of technique varies depending on the pathology, the application area, the structure of the dermis and the type of product. Frequently, agents are applied from the epidermis to the hypodermis.

Benefits of Mesotherapy

Due to the method of application of mesotherapy, the drugs reach the problem area directly. Therefore, mesotherapy is a faster and more effective treatment method. Disintegration of drugs and failure to reach the problematic area, which can be encountered in other treatment methods, are not seen in mesotherapy. Mesotherapy is a non-painful treatment that leads to skin regeneration and is applied with very small needles. As a treatment method with strong effects such as slimming, skin renewal and healing, mesotherapy has no known harm. There may be only allergenic conditions. For this reason, it should be well investigated whether people are allergic to the agents and a suitable process should be followed.
